Requesting an Appointment
Appointment requests can be submitted through our website or by calling or texting our front office staff at (805) 801-2231. Client email, date of birth and credit card must be held on file to secure an appointment time.
Cancelation Policy:
All appointments must be canceled 24 hours in advance, or a $50 cancellation fee will be charged to the credit card on file. After three missed appointments or same day cancellations you may or may not be rescheduled with your therapist.
Insurance
We are currently in-network with these insurance carriers:
Medicare Aetna CenCal SaNa CalVCB
If you would like to use these insurance carriers, complete the Client Insurance Form in your patient portal after you’ve been scheduled. Our office will verify your insurance is active and provide a good faith estimate of your deductible and co-pays. If your insurance lapses or does not cover services, you will be responsible for charges.
Other accepted forms of payment:
Health Savings Account (HSA):
If you would like to use your HSA enter that account information into the credit card form available on the client portal.
Worker’s Compensation:
Worker's Compensation contracts will be considered on a case by case basis.
Direct Pay:
We accept debit cards, credit cards, cash and checks at the time of appointment. Our office uses auto-pay, allowing you to keep your card on file for automatic payments after each appointment.
Out of Network Insurance:
If we are not in network with your healthcare provider we will charge your credit card on file for sessions and provide you with a superbill to submit to your insurance carrier.
